It goes like this:
Respeccing
Hell Diablo/Meph/Baal have a chance to drop Essence of Terror/Hatred/Destruction, when all 3 are collected and combined in the cube, you get a respec token. This can be done as many times as you wish.
Pandemonium Event
Hell Andy/Duriel/Nihiliak have a chance to drop keys of Terror/Hatred/Destruction or something like that, when all 3 are collected and combined in the cube you get a portal to a random Uber boss. The boss being an upgraded version of Andy/Duriel/Nihiliak. This has to be opened in Act 5 Hell.
Each of the uber bosses drops another token, when all 3 are collected and transmuted in the cube, you get a portal to Uber Tristram. Uber Tristram is home to Uber Meph/Baal and Pandemonium Diablo, this is where you get the class torches. I think it drops on the last boss killed but is difficult because they have a lot of health and a lot of spawned minions. You can only hold one class torch at a time, no matter what class it is. Also has to be opened in Act 5 Hell.
You can open all 4 portals in a single game, but you will need 3 sets of keys to do so, you cannot open up a portal to the same boss twice in a game. So 3 sets of keys will get you all 3 bosses and ultimately Uber Tristram.
Uber/Clone Diablo
This version of Diablo is much tougher than Pandemonium Diablo. Uber Diablo takes the place of a random Super Unique in any Act only on Hell difficulty and is the only mob in the game who can drop the Annihulus charm. If I remember right it's a small charm with +skills and mf and good stats. You will get the message, "Diablo walks the earth." when he is in your game. I read a thread once describing how to hunt him and it has something to do with the number of sojs sold to vendors as well as ip and some other weird shit. I've only seen it once in the entire time I've been playing the game. I couldn't kill him.
