Fast-food places that don't deliver: I don't tip, seeing as how 1) most fast food workers are horrendously incompetent and 2) fast food salary is, from what I understand, above minimum wage, which means that tipping isn't justified.
Delivery restaurants: A typical pizza order usually gets a $2 tip on principle, though larger orders make me bust out my percentages. Other delivery places always get percentage-based tips, though.
Shops (convenience stores, ice cream parlors, etc.) with tip jars: If the service was above-average and the employee was pleasant, I'll throw in my spare change. And if the worker is absolutely amazing---this usually only applies to, like, ice cream shops---I'll occasionally throw in an extra single or two.
Small restaurants/diners: 15% is my standard. Mediocre service results in a 10% tip, poor service results in no tip and great service warrants 20%. I've had a small handful of servers get 25% because they were so fantastic, though. But it's really just a matter of disposition on both sides.
Larger, fancier restaurants: 10% is my baseline, seeing as how the employees are making so much from tips anyway. If the service is above average, it goes up to 15%. If the service is phenomenal, 20%. And if I get a handjob under the table from the waitress while she screams into my ear the lyrics to Whitesnake's "In the Still of the Night," 25%.
