Depends if they are permitted by law and their ToS to scan your realmlist.wtf file. That is the only file you modify. Having TBC or Vanilla installed may not be commonplace, but it's not exactly suspicious, and it's definitely not enough to action an account.

Even if they can tell the file has been modified, they cannot tell to what extent.
