(forgive me if the quest progression is wrong as I haven't done the quest myself and only seen it done)
Let's see... there seems to be a problem around the time of tossing item into yogg saron mouth... Then Isamoo gets screwed over by GMs not refunding fragments (
IF HE DELETED THEM THEY COULD'VE SEEN ALL 30 AND RESTORED ALL 30)
So, my thoughts would follow this logically as. There seemed to be some issue with Isamoo's quest line causing it to not think he had 30 shards, which also appeared when GMs were attempting to look at logs. IIRC when he spoke to a GM they said they had corrupted logs from certain dates and couldn't award fragments for those days because they weren't sure.

Except that's not how it works.
When you get 30 [item]Fragment of Val'anyr[/item], you can combine them into the [item]Shattered Fragments of Val'anyr[/item]. This item starts a quest, which ends at the Archivium Console. The console then gives you a NEW quest, for which [item]Unbound Fragments of Val'anyr[/item] - what you actually throw into Yogg's mouth - is provided.
Like any such quest with a provided item, if the item were to disappear, he could drop the quest and get another one.
If the GM restored some frags and he made another quest item, if he attempted to use it, it would give the Quest Completed message.
His story is not consistent with the mechanics of the quest.
