I also wonder if they had to take anything out to refrain from hitting an AO rating.

And if you're wondering why they'd cut things, they would for the same reason they cut things in movies to drop from NC-17 to R:

So places of sale/viewing will accept them.

it's hard to say. i know they cut something out of the end of the trailer, but that could be just so they can show it. i can't imagine them doing anything out of the actual game though since duke 3d didn't hold back much at all. WOULDN'T FEEL LIKE DOUK.
